Here's a screenshot of what the options panel looks like with background monitoring. It's coming along, and it mostly seems to work. I'm sure there are still kinks to work out and only time figures those out. Otherwise there are still a few other major things to do, namely, I need to figure out to minimize iTunesDSM to the system tray, that way when it's backgrounding it, it won't be on the start menu.
